The Certificate of Records Destruction (RM3 form) Excel version was introduced in September 2023. Thanks to everyone who provided feedback on the new form. We have made small corrections/updates to the form and are ready to launch it as the official UVA RM3 form. We are permanently retiring the Word version of the form. Going forward, please submit the Excel RM3 Form to request approval to destroy university records.
